SURRENDERING OR REHOMING A DOG

Goofy Foot Dog Rescue is a foster-based program out of West Tennessee. We do not have a shelter or any type of public facility. All of our dogs are fostered inside of our homes, unless they are part of the PADDLE program. All foster homes are routinely inspected to ensure the dogs are being cared for adequately.

All of the dogs that enter our program will be fully vetted. Fully vetted means they've been to the vet, had age appropriate shots (2 sets if the dog is a puppy), rabies vaccine if over 12 weeks, fecal, deworming, heartworm test if over 6 months, spay or neuter, grooming, flea/tick/heartworm prevention is current and any other vetting that the dog may require.

Please understand that Goofy Foot Dog Rescue can only accept new dogs into our program as space allows. If we do not have an open spot in a foster home or in the PADDLE program, we cannot take responsibility for that dog.

If Goofy Foot Dog Rescue is able to accept this dog into their foster care program, I release ownership of my dog to Goofy Foot Dog Rescue. I understand that upon giving my dog to Goofy Foot Dog Rescue, they become the sole owner of the dog. I will make no attempt to reclaim my dog. I fully grant Goofy Foot Dog Rescue the right to give my dog the medical treatment deemed necessary and to place the dog in an adoptive home. Should a veterinarian determine my dog is too ill to be treated, if necessary, I give Goofy Foot Dog Rescue permission to take all steps necessary to treat this animal, which can include euthanasia. I release Goofy Foot Dog Rescue from all liability concerning said animal. Goofy Foot Dog Rescue will make every attempt to provide adequate care and find a permanent home. I hereby state that I am giving up my dog of my own free will. *

I understand that submitting this application does not guarantee that Goofy Foot Dog Rescue can accept this dog into a foster home. Goofy Foot Dog Rescue is a foster based rescue and can only take in owner surrenders/strays as space allows. *
